Forest City Volleyball Club Registration Information

Please bring the following with you to registration:

  1. FCVC Athlete Registration Form - ensure that your Health Card number and any health concerns are recorded on this form
  2. OVA Registration form
  3. FCVC Parent Volunteer Information Form
  4. FCVC Treasurer's Form
  5. Parent Code of Conduct - signed by parents or guardians
  6. A copy of your birth certificate if you are new to the club
  7. Registration fee of $625 in full or by three instalment cheques as follows:
    1. $220 dated October 29, 2008
    2. $220 dated November 15, 2008
    3. $200 dated December 15, 2008
  8. $150 cheque for raffle tickets
  9. Please attach all cheques to the Treasurer's Form

All cheques are made payable to Forest City Volleyball Club.
