Welcome to the Forest City Volleyball Club Registration page and congratulations on your successful tryout with the club for the 2007-2008 season.

Registration Materials:

All registration materials including a general information newsletter, the FCVC athlete information form, the FCVC Parent Volunteer Information Form, the OVA registration form, and the Parent Code of Conduct are available under Registration Forms. Successful athletes will be responsible for completing and printing these forms and bringing them to the registration nights (see below).

FCVC Superstore:

On registration evenings, athletes will have the opportunity to purchase optional FCVC clothing and bags, and other clothing from Adidas, all at reduced prices. More information about these items is posted in this section under Volleyball Equipment. Orders can be placed until December 1, 2007.

Uniforms:

Included in your registration fee is an upgraded uniform and warm-up jacket from Adidas. Please note that if a uniform shirt is lost during the season it will be the responsibility of the player to replace it at a cost of $50.

Volunteer Opportunities:

The success of FCVC relies on the support and participation of parent volunteers. Available positions are outlined in the Responsibilities and Expectations newsletter. These positions include a Team Parent Manager, Social Representative and a Net Income Group representative. If your team elects to run a concession to raise team funds then parents will be expected to contribute their time. FCVC parents will not volunteer at bingos this year and they will not submit a $100 "bingo" cheque as was the practice in the past.

FCVC NET Income Group:

The FCVC Raffle is a new fundraising initiative introduced this year to offset the loss of bingo revenue. In addition to the registration fee, all athletes must purchase 30 raffle tickets ($5 each). Parents and athletes will be able to recoup the expense of these tickets by selling them in the community. The NET Income Group will support teams with instructions and ideas for fundraising projects. A parent volunteer from each team will coordinate the distribution of raffle tickets and the collection of raffle ticket stubs.

Registration Fees:

Fees (13U to 18U) for the 2007-2008 season are $625. This fee can be paid in full on registration night or in three instalments payable as follows: $220 on registration night, $220 on November 15, 2007 and $200 on December 15, 2007. A $15 administrative fee will be charged when paying the fee through the instalment plan. The FCVC registration fee includes registration with the OVA, a full uniform, practice facilities and tournament fees.

Registration Process:

Please read all material posted on the website.

Athletes must complete and print all registration forms and bring them to registration night. A parent or guardian must accompany the athlete to registration and sign all forms as necessary. Uniforms will be sized and ordered that evening and athletes will also have the opportunity to visit the FCVC Superstore.

Please bring the following with you to registration:

1. FCVC Athlete Registration Form - ensure that your Health Card number and any health concerns are recorded on this form
2. OVA Registration form
3. FCVC Parent Volunteer Form
4. Parent Code of Conduct - signed by parents or guardians
5. A copy of your birth certificate if you are new to the club
6. Registration fee of $625 in full or by three instalment cheques as follows:
1. $220 dated on your registration night
2. $220 dated November 15, 2007
3. $200 dated December 15, 2007
7. $150 cheque for raffle tickets

All cheques are made payable to Forest City Volleyball Club.
